Hi and thankyou for allowing me to join. I was diagnosed around seven years ago with Barretts. Since original diagnosis I have only had one surveillance scope where it was determined that although I did have additional areas of Barretts, they were very small and I was "signed off" by my consultant and told that unless I had further bother I would not receive any further scopes. I take 30mg Lansoprazole twice a day and Gaviscon at night which the consultant told me I should stay on for life. I had a pretty bad flare up recently and asked my doctor to be referred back to Gastro Clinic which he did but both the referral and the scope was denied. I had constant chest discomfort and palpitations and pain in my back also. I would describe it as a feeling of anxiety as if a panic attack was imminent. However, the worst of that seems to have passed now but I have been left with an "itch" in my throat when I Laugh that always results in a coughing fit. the itch is also there sometimes when I swallow fluids. Can anyone tell me if this is something that's normal with Barretts?I am really concerned that as I have had no checks in 4 years now that things may have got worse. How would they know if I don't get checked.this is worrying me constantly now. Many thanks in advance for you support.

Hi and welcome to the forum.

Because there is always a risk of damage when scoping and the vast majority of scopes do not show anything new, many with very small areas of Barrett's are no longer routinely scoped.

The symptoms you describe are consistent with reflux. Your lansoprazole will be reducing the acid to make such reflux less damaging but they don't actually reduce the reflux itself, as concluded in this paper.

Discomfort in the chest and back occurs from spasm in th eoesophagus impinging upon the vagus nerve as it passes through the hiatus hole in the diaphragm.
The "throat itch" and coughing can also be concomitant with Extra-oesophageal reflux.

To reduce reflux, you need to follow the
lifestyle modification advice.

I didn't think size mattered? What is classed as very small,... 1cm or less? I always thought the tiniest Barretts could progres.   Kind of a lottery if gastros are just dismissing small segments no?

Any individual cell has the ability to mutate albeit extremely small risk.
The more cells though, the greater chance that one of them will mutate.

thankyou so much for your informative reply and the links you have supplied. It has eased my mind considerably. Wouldn't it be so much easier if this information was made clearer to patients at the time of diagnosis. I have one more small question that you may be able to help me with. Very randomly I can suffer a whole day of severe heartburn even though I haven't forgotten to take meds etc and with no other apparent reason, have you any idea what may cause this so that I might be able to prevent it? Otherwise I never have heartburn any more.Thank you again
