After just being diagnosed recently with Barrett's and stopping Nexium 40mg daily after two weeks because of indigestion and cramps etc, i have a few questions that maybe someone could help me with;

I was advised to take Nexium 40 mg for 8 weeks and then to go onto a lower maintenance dose after that indefinitely. I do understand that PPI treatment lowers the risk of cancer down the line and prevents and helps with the symptons. What i don't understand is what is the reason behind advising that new patients diagnosed with Barrett's need to take an aggressive high dose for 8 weeks after diagnosis?

What is the purpose of this initial high dose and what does it hope to achieve?

The normal advice given to all is high dose for a few weeks and then maintenance dose.
The idea is, newly diagnosed you are likely to have oesophagitis which the higher dose for a few weeks will help heal.

I wasn't told to take a high dose after my inital diagnosis 3 weeks ago (although I still don't have the biopsy results). I was put on 20mg omeprazole per day. Is that ok?

20mg maintenance dose is quite OK. High initial dose is not mandatory but may be prescribed when the possible presence of oesophagitis is considered and necessary to be eradicated.
If your 20mg is not proving sufficient to reduce your acid symptoms, you may discuss doubling for a few weeks.
